Athlete eligibility must be valid from the 1st day of practice through the last day of competition to participate in any practice or competition for that sport.
Establish or Renew Athlete Eligibility – Once Every Three Years
Athlete Eligibility expires three years after the Physical Exam date on the Athlete Medical Form.
- Complete all parts of the Athlete Medical Form
- Athlete Medical – Health History pages 1 and 2
- Athlete Medical Form – Physical Exam
- Athlete Medical Form – Medical Referral (if needed)
- Complete the Athlete Participation Waiver.
- Complete the Communicable Disease Waiver.
- Send the completed Athlete Medical, Athlete Participation Waiver, and Communicable Disease Waiver to medical@somdmontgomery.org as a single PDF document.
Please submit all forms in a single message as one or more PDF files* with original signatures. Send your PDF(s) at least two weeks before the first practice to allow time for processing. Incomplete or partial submissions cannot be processed and will not be saved.

- When your sport’s Sign-Up opens, sign-up for the sport at Sport Season Signup – Athletes.
Please do not bring your forms to practice. Coaches can no longer accept forms at practice; all forms must go through our medical team for processing.
